Good provocative piece. The lack of systems thinking is evident in all the large companies I work with and the rise of complexity is a factor but you are right that it is about leadership and governance. But it is also something else--failure in collaboration.

The current administration&#039;s response is a sad example. BP needs to pay but this needed collaboration and a win win. It has become a lose lose. 
Imagine if the Commander in Chief and the Heads of BP sat down on day one and figured out how to use our deep water capabilites /expertise and BPs capabilities to get on this asap? i.e. Deep water subs with robot arms could not help with this?
Instead, Lets send in the lawyers and assign blame.    

I know you would agree, that without true collaboration, system thinking and leadership alone will not prevent or solve very much. 
As for exec search, this is very relevant, the systems thinking, the leadership and the collaboration always yield the best choices.
